REHABILITATION
Activate O.T is able to provide an independent functional, holistic assessment of your needs, recommend and carry out treatments and interventions to improve independence and safety. All intervention is based around your own individual and personal goals and all within your own home.
The aim of rehabilitation is to improve your ability to carry out day to day activities including work and leisure activities that have been effected by an illness or injury. We provide one to one home based assessments. We set goals guided by you, working with you in your own home or place of work to achieve these goals.
Leisure rehabilitation- Being socially isolated and unable to engage in meaningful and enjoyable activities can significantly affect individuals physical and mental health. We can work with you to enable you to take part in leisure/ social activities and hobbies that you may have become isolated from due to a health condition.


ASSESSMENT / EQUIPMENT PROVISION
Activate O.T can assess for and provide specialist equipment that is appropriate for your needs and abilities. Equipment ranging from low level bathing, toileting, seating, eating and drinking, mobility aids, specialist seating, moving and handling (including hoisting) and mobility aids.
All assessments are carried out within your own home.
A full report and recommendations will be provided following your assessment. There is no obligation for you to purchase the recommended aids or equipment or undertake the recommended treatment.
A Full breakdown off all costs of equipment will be provided in your report which will be provided prior to any equipment being ordered/purchased/hired. Equipment provision or further interventions will only take place with clients’ consent.
FALL PREVENTIONS AND HOSPITAL ADMISSION PREVENTIONS
Activate O.T can work with individuals in their own homes, with individuals in residential care and sheltered accommodation. We provide group falls prevention classes and education.
Falls are the most common cause of death in the over 65’s making this a crucial area for preventative action.
By taking into account your environment, daily occupations and physical abilities we can help you minimise your risk of falls. We will assess your home environment and advise on home modifications or equipment, address any fear of falling, discuss common causes of falls and advise on how to incorporate activities to improve strength and balance into daily occupations and routines.
