ACTIVATE OCCUPATIONAL THERAPY RATES & TERMS AND CONDITIONS

A typical initial assessment takes from 90 to 120 minutes for the face to face assessment however this is dependent on complexity of the case. 

​

You should expect to receive your typed assessment document within 3 working days of your assessment. 

​

A treatment plan will be provided prior to the commencement of any ongoing work to give a full breakdown of costings/timings to be expected. 

​

Additional/ Ongoing interventions including rehabilitation, treatments and follow up visits, joint visits with reps for equipment etc will be charged at an hourly rate.

Mileage is only added at 50 pence per mile for cases outside of 30-minute radius from base). 

​

Individual payment plans for continuing therapy can be discussed prior to commencement of treatment. 

​

Clients are under no obligation to continue with any treatment and can end treatment at any time. 

​

Payment options include- Cash, Cheque, and Direct bank transfer. 

​

Invoiced payments to be paid within 7 days upon receipt of invoice. However, if aids or adaptations are required this will require payment upfront. 

​

Normal working hours are Monday to Friday 9am-5pm and above rate will apply. 

​

Clients will be expected to provided their contact details and that of their GP and next of Kin in case of Emergency. They will not be contacted unless felt appropriate by the Occupational Therapist.

Confidentiality will be adhered to at all times and in line with data protection policies. However, the Occupational Therapist withholds the right to contact appropriate agencies if it is deemed that the client is a risk to themselves or others.  

​

Zero tolerance policy is in place with regard to any verbal or physical abuse.
